2020-2021 Meal Pick-up Form

This form is for parents/guardians of Walton-Verona School Students that are on Virtual/Hybrid and Friday NTI and who may be interested in receiving meals at no cost (breakfast and lunches) for their enrolled students or any child 2-18 years of age that live in your household that wishes to receive meals.

We plan to provide 5 meals for Virtual students, 2 meals + 1 NTI meal for Hybrid for a total of 3 meals, and 1 meal for all NTI Friday students. The Meal Pick up will replace the Meal Delivery that has taken place since August 24th and anyone interested will need to fill the New Meal Pick-up Form out and submit it.

To meet federal regulations we are required to track how much food we prepare. Our first pick-up is planned for September 17th, 2020 between 4:00 p.m. and 6:00 p.m. at the High school bus loop in front of the High school so it will be necessary that this form is completed and submitted by September 15, 2020, to start receiving food. Please note that you can opt-in for meals after September 15th but your next food pick-up would be the following September 24th.

These meals will come with a menu and cooking instructions for each day, along with preparation and serving instructions.

Completion of this form indicates that you would like to be a part of the Meal Pick-up for the child(ren) named below. Please note: You need to enter each child and choose which option that child will receive. For a non-student in the household between the ages of 2-18 that you wish to receive meals you just enter a # and they will receive 5 meals. After completing the form click on the Submit button.

Walton Verona Schools Food Service Program is operating Under USDA’S waiver extensions at this time and all students in the District (In School, Virtual, and Hybrid) including non-students ages 2 – 18 years will receive Free Meals under the Summer Food Service Program until this waiver expires.

You may put money into accounts using Parent Portal for IC, sending in a check to school, or dropping off cash or check to the front office between 9:00 AM-12:00 PM.
